I am not interested in working with Piper or Animal Assisted Therapy. Can I schedule an appointment without Piper present?
Absolutely! When you schedule your appointment, please let me know if you would like Piper to be present. She will be working with me specified days only.

How are appointments handled at GreeneCare?
Your first appointment is generally an initial evaluation, which lasts approximately 50 minutes. This will allow enough time to gather information about why you are seeking assistance, and assure that you have ample time to ask questions. You may be asked to arrive early in order to fill out initial session paperwork or this paperwork can be completed online (see below). Follow up appointments will ordinarily be 50 minutes (one appointment hour of 50 minutes) in duration, once per week at a time we agree on, although some sessions may be more or less frequent as needed.
Cancellations and Missed Appointments
If you need to cancel or reschedule a session, I ask that you provide me with 24 hours’ notice. If you miss a session without canceling, or cancel with less than 24 hour notice, my policy is to collect the amount of your co-payment (unless we both agree that you were unable to attend due to circumstances beyond your control). It is important to note that insurance companies do not provide reimbursement for cancelled sessions; thus, you will be responsible for the portion of the fee as described above.
The standard fee for the initial intake is $125.00 and each subsequent session is $115.00. You are responsible for paying at the time of your session unless prior arrangements have been made or we agree otherwise. Payment must be made by check, credit/debit card or cash. Any checks returned to my office are subject to an additional fee of up to $25.00 to cover the bank fee that I incur.
Can Insurance Pay for my session?
If you have a health insurance policy, it will usually provide some coverage for behavioral health treatment. With your permission, I will fill out forms and provide you with whatever assistance I can in helping you receive the benefits to which you are entitled; however, you (not your insurance company) are responsible for full payment of fees owed. It is very important that you find out exactly what behavioral health services your insurance policy covers. You should carefully read the section in your insurance coverage booklet that describes behavioral health services. If you have any questions about the coverage, call your plan administrator.
What paper work do I need to complete?
This form provides the background physical and mental health information needed by the clinician. You may fill it out ahead of time and bring it into the office on your first visit.

The Notice of Privacy Practices form describes how your personal health information will be safeguarded and under what circumstances it will be shared.
In the event that the patient is a minor, the following form must also be completed.
